As a Child Case Manager you will be providing "home-based" treatment and "community-based" services through: case management, skills training, psycho-education and professional support focused on assisting clients to access needed medical, social, educational and other local community-based services.

Responsibilities- Provide case management, IROS, and IMR services in both the client's home and in the community
- Assist clients in managing daily problems and stressors
- Assist with developing new life structure via home, school or other meaningful activities
- Develop life and illness management skills and family education and support
