Trillium Health Resources is seeking a 1915(i) Triage Coordinator to coordinate services for members enrolled in the 1915(i) program. You will complete required assessments, connect members to internal teams, and ensure smooth transitions and supports.
Based on diagnosis, eligibility, and service needs, you will identify appropriate referrals and coordinate with LRPs to keep assessments up to date and responsive to member needs, with remote work options within NC.
