Advanced Practice Provider

Harmony Cares seeks an Advance Practice Provider to deliver home-based primary and palliative care to medically complex, home-limited patients. In this role, you will conduct in-home and telehealth visits, perform comprehensive assessments, manage chronic conditions, and coordinate interdisciplinary care to reduce avoidable hospitalizations. You will collaborate closely with physicians, nurses, social workers, and care managers in a compassionate, patient-first culture focused on access, outcomes, and dignity.
