Fletcher Rehabilitation and Healthcare Center is seeking a highly skilled MDS Hybrid Position to join our team as a Hybrid MDS Coordinator. This full-time role blends onsite and remote work (3 days onsite, 2 days remote) to coordinate and manage nursing care plans for our residents.
You will ensure accurate MDS documentation, maintain regulatory compliance, audit records, and support discharge processes.
Required:
current state nursing license and CPR certification; preferred: 1 year in
