Reema Health is seeking a Community Health Navigator to engage with community members across Indianapolis and surrounding areas. You will build relationships, guide members toward greater engagement in their healthcare, and work through Reema's technology platform to reach members, schedule visits, and document progress.
You’ll manage a caseload of 80–120 members, coordinate referrals, and help members overcome barriers with empathy and timely follow-through.
