RoleSummary
The Care Navigator supports patients by coordinating referrals, scheduling appointments, and resolving barriers to care. Operating in a fully remote capacity, this role conducts telephonic and digital outreach, educates patients on next steps, and ensures referral loop closure within defined SLAs. Success requires empathy, attention to detail, and a proactive approach in a fast-paced, virtual environment dedicated to patient advocacy and operational excellence.

Key Responsibilities- Patient Engagement & Advocacy: Conduct timely outreach via phone, text, and email to schedule appointments and provide culturally responsive education to reduce no-shows.
- Referral Management: Prioritize referral tasks to meet SLAs, coordinate with specialty providers, and track statuses to ensure successful loop closure.
- Data Integrity & Compliance: Document all interactions accurately in EMR/CRM systems, adhering to standardized templates and HIPAA regulations for audit-ready records.
- Operational Excellence: Monitor personal queues and dashboards to prevent delays while following internal QA workflows to ensure high-quality patient outcomes.
- Bachelor’s degree in Social Work, Healthcare, Communications, or a related field.
- 5+ years of professional experience in social work, medical office management, or a specialized patient services role, with a demonstrated focus on referral coordination and navigation.
- Ability to work in a fully remote capacity from a private, HIPAA-compliant workspace with reliable high-speed internet.
- Ability to work the required shift of 11:00am – 7:30pm EST.
- Experience with patient scheduling and referral coordination.
- Comfortability using EMR/EHR, CRM, and telephony tools.
- Working knowledge of HIPAA/PHI regulations and data privacy standards.
- Previous experience in a fully remote work environment.
$23.00 - $24.00 per hour + full benefits and 401(k).
