• Educate patients and caregivers on regimens and disease-state management.
• Document case activities and outcomes in compliance with health-plan and regulatory standards.
• Participate in quality initiatives aimed at reducing hospitalizations and improving outcomes.

• Education:
  
  PharmD or Bachelor of Pharmacy.
• Experience:
  
  Case management, MTM, or managed care preferred. Hospital, ambulatory, and community pharmacists with strong counseling backgrounds are encouraged to apply.
