Activate Care is seeking a Hybrid Care Coordinator in Nevada to support screening, assessment, and care navigation for Path Assist participants. The role combines work-from-home tasks with field visits within the local designated area.
You will help clients access services, create care plans, and coordinate with local nonprofits to improve health outcomes. The ideal candidate has CHW credentials (or willingness to obtain), a valid Nevada driver’s license, and 2–3 years of direct care
