Koinonia Family Services in Tustin, CA is seeking a part-time ECM Care Coordinator to join our Enhanced Care Management team. You will assess needs, develop care plans, coordinate services, and connect youth and families to essential resources, with in-home visits as needed.
Hours:
20–25 per week, with some remote work between in-person sessions. The ideal candidate speaks Spanish, has at least two years of case management experience, and is comfortable working with high-needs populations.
