Bloom Healthcare in Texas is seeking a Hospice Social Worker to join our home-based hospice team. You will perform psychosocial assessments, develop plans of care, provide counseling, and connect families with resources. This role requires driving and visits in Live Oak and Hollywood Park, with weekend availability.
Ideal candidates hold a Master’s in Social Work with 1–3 years in palliative care and strong communication. A CO driver’s license is required.
