HSS Clinical Coordinator, RN - Omaha or Lincoln, NE - Remote

About This Role

In this Health and Social Services (HSS) Clinical Coordinator, RN role, the individual will be an essential element of an Integrated Care Model by relaying pertinent information about member needs and advocating for the best possible care available.

• Location: Omaha, NE
• Department/Unit: Health and Social Services (HSS)
• Schedule: Full Time, Day shift
• Residency: Must reside in or within commutable driving distance to Lincoln and/or Omaha, Nebraska
• Work Style: Remote with field visits

• Assess, plan and implement care strategies individualized by patient.
• Identify and initiate referrals for social service programs.
• Manage the care plan throughout the continuum of care.
• Communicate with all stakeholders to ensure quality coordinated care.
• Advocate for patients and families to ensure their needs are represented.
• Spend about 25% of time in the field visiting members in their homes or long-term care facilities.

• Unrestricted and independent RN license in the state of Nebraska

• 4+ years of experience working within the community health setting or in a health care role

• Valid driver's license and access to reliable transportation
• Proven bilingual in English and Spanish
• Resident of the state of Nebraska
