Job Overview

The Care Coach 1 assesses and evaluates members’ needs and requirements to achieve and/or maintain optimal wellness by guiding members and families toward appropriate resources and facilitating interactions with providers and interdisciplinary teams. The role reports to the Regional Care Coach Manager and is based in Hillsborough County, Florida.

• Visit Medicaid members in their homes, assisted living facilities, long‑term care facilities, and other care settings (75–90% local travel).
• Assess and evaluate member needs to establish individualized care plans.
• Ensure members receive services in the least restrictive setting to achieve optimal well‑being.
• Plan and implement interventions to meet those needs.
• Coordinate services, monitor, and evaluate case‑management plans against personal goals.
• Guide members/families toward resources appropriate for their care.
• Facilitate interactions with payer sources, providers, interdisciplinary teams, and other involved parties as required by the comprehensive contract.

• Bachelor’s degree in health and human services or related field.
• Two or more years of related experience.
• Proficiency with Microsoft Office Word and Excel.
• Exceptional verbal and written communication and interpersonal skills.
• Ability to use multiple electronic information applications and software programs.
• Must be screened for tuberculosis (TB) as part of the role.
• Valid state driver’s license and personal vehicle liability insurance meeting specified limits.
• Separate room with a locked door for a home office to ensure privacy of member information.

• Experience with electronic case‑note documentation in multiple systems.
• Experience working with Medicare and Medicaid recipients.
• Experience in health promotion, coaching, and wellness.
• Knowledge of community health and social service agencies and additional community resources.

• The role is patient‑facing and part of Humana’s Tuberculosis (TB) screening program.
• The role is part of Humana’s driver safety program; employees must maintain appropriate vehicle insurance.
