The SE (Southeastern Mental Health Authority) (SMHA), under the umbrella of the State of Connecticut, Department of Mental Health and Addiction Services (DMHAS) is seeking a motivated and detail-oriented Mental Health Assistant 2 (PCN 26213) to join our Young Adult Services team in Norwich.   • First shift, 8:00 a.m.

WHAT YOU WILL DO:
  • Provide case management services for 60+ young adults in the Young Adult Services (YAS) Program, supporting housing, entitlements, transportation, and access to essential appointments.
  • Assist young adults in accessing community resources that support basic needs, safety, and independent living.
  • Participate in assessments, service linkage, outreach, crisis intervention, and home-based family contacts.
  • Teach and model independent living skills; provide targeted case management interventions to an assigned caseload.
  • Monitor clients’ self-administration of medications and coordinate outpatient appointments.
  • Serve as a liaison to community agencies and maintain communication with the clinical treatment team.
  • Enter documentation in the electronic health record, including recovery plans, reassessments, encounter notes, and required case management records.
  • Co-lead psychosocial rehabilitation groups and provide verbal support and problem-solving assistance in community settings.
  • Support high-risk young adults with complex psychiatric, medical, trauma, legal, and substance use challenges transitioning from youth services to adult living.
  • Provide Targeted Case Management, a Medicaid-billable service, ensuring compliance with billing and documentation standards.

The Southeast Mental Health Authority is a community mental health agency whose primary goal is to provide high quality behavioral health services to those in our community with mental health and substance use disorders who are 18 years or older and reside in New London County and its surrounding towns.

Services, which include case management, outpatient behavioral health, crisis intervention, respite care, and forensic services, are recovery oriented and offered in a trauma informed and culturally sensitive manner. We support individuals to identify and achieve their own goals in recovery, fostering independence, dignity, and ability. As a State Operated Local Mental Health Authority, SMHA is responsible for administering all DMHAS contract funded behavioral health services in southeastern Connecticut.

PURPOSE OF JOB CLASS (NATURE OF WORK)

In a Department of Mental Health and Addiction Services hospital, community behavioral health facility or program this class is accountable for leading Mental Health Assistants in the assessment, planning and implementation of paraprofessional nursing, clinical and/or rehabilitative interventions designed to support client short and/or long-term needs. In outpatient or community programs this class is accountable for independently performing the full range of paraprofessional functions in case management and crisis intervention services.
