The Case Manager is a full-time, hybrid role based in the Greater Seattle Area, with a combination of in-person visits and some work from home. This role involves conducting assessments of older adults’ medical, functional, and psychosocial needs and developing individualized care plans. The Case Manager will coordinate services such as home care, medical appointments, and senior living placements, while maintaining regular communication with clients, families, and providers.
Daily responsibilities include documentation, care plan updates, monitoring client progress, and advocating for appropriate resources and interventions. The Case Manager will collaborate closely with nurse care managers and other team members to ensure continuity of care and high-quality client outcomes.
- Strong skills in care coordination, case management, and client advocacy, preferably within elder care or senior services.
- Proficiency in conducting assessments, developing care plans, and monitoring outcomes for older adults.
- Effective communication, interpersonal, and relationship-building skills with clients, families, and interdisciplinary teams.
- Organizational and time management skills, including accurate documentation and follow-through in a hybrid work environment.
- Comfort using electronic health records, scheduling systems, and basic office technology to support remote and on-site work.
- Experience working with older adults, caregivers, or in healthcare, social work, or related fields; familiarity with local senior resources is a plus.
- Ability to travel within the Greater Seattle Area for client visits and meetings as needed.
- Relevant professional licensure or certification (e.g., RN, LPN, MSW, or gerontology-related credential) preferred; equivalent experience considered.
