Banner Health's Readmission Case Management team seeks a High-Risk RN Case Manager. You will manage a caseload of 40–60 members, collaborating with LMSW, Pharmacy Team and Registered Dietitian to coordinate care across ambulatory settings. Telephonic, home visits and televisit platforms are used.
Hours are Monday–Friday, 8:30 AM–4:30 PM; remote with limited in‑person visits in AZ. Applicants must hold a BSN, an active RN license in the state, and BLS certification.
