Care Manager

Position Overview

Centene is looking for a Care Management Specialist to work with our 28 million members. This remote position is open to candidates residing in Missouri.

• Develop, assess, and facilitate complex care management activities for members with mental and behavioral health needs, providing personalized care plans and education for members and families.
• Evaluate member needs via phone or in-home visits, recommend or facilitate care plans/services for the best outcomes, addressing behavioral health and social determinant needs.
• Perform telephonic, digital, home, or other site visits to outreach and assess member needs, collaborating with resources.
• Develop ongoing care plans for high-acuity members, identifying providers, specialists, and community resources.
• Coordinate among members, caregivers, community resources, and care provider teams to ensure services are accessible.
• Monitor care plans, member status, and outcomes for changes in treatment side effects, complications, and clinical symptoms, providing recommendations based on identified needs.
• Facilitate care coordination and collaborate with providers or specialists to ensure timely access to needed care or services.
• Collect, document, and maintain member information and care management activities to ensure compliance with state, federal, and third‑party payer regulations.
• Provide education to members and families on procedures, provider instructions, treatment options, referrals, and benefits, including behavioral health and social determinant needs.
• Provide feedback to leadership on opportunities to improve and enhance care quality and delivery for members cost‑effectively.
• Perform other duties as assigned and comply with all policies and standards.

Education:

A Master’s degree in Behavioral Health or Social Work, or a degree from an accredited School of Nursing, and 2–4 years of related experience.

License/Certification:
Licensed Master’s Behavioral Health Professional (e.g., LCSW, LMSW, LMFT, LMHC, LPC) or RN, based on state contract requirements, with behavioral health experience.

Compensation: $56,200 – $101,000 per year. Actual pay will be adjusted based on skills, experience, education, and other job‑related factors.
