Job Title
Job Type: Full time In-Person Schedule - Monday to Friday, 7:00 Am to 3:00 Pm Pay: $24.00/Ph

Working with adults who have intellectual disabilities to accomplish a goal depending on where they fell on the scale. Facilitating two buildings with 16 beds each. Making sure each building is properly staffed and stocked of items needed to take care of the residents. Ordering food and supplies for both homes. Making sure each resident charts are updated with care plans.
Organizing doctor appointments, supervising DSPs and listening to concerns of the residents' guardians families.
Daily interacting with the individuals to ensure their goals are met and to monitor their behavior patterns and to assist with their mental and social support daily and prepare individual goal support report monthly as needed.
Liase on behalf of our DDD individuals with the behavioral therapist the counselors the guardian and other peers in the home to ensure they are comfortable with environment support and personal goals assigned
Conflict resolutions between peer to peer and staff to peer as the case may be and filing necessary CIRAS report with the state as the case may be.
Interact with our DDD clients and prepare their job behavior plans and monthly reports for the review after Residential Director
Supervise the Group Home Workers around issues pertaining to daily living activities of all the residents.
Supervise the Group Home Workers in the integration of each resident's counseling/ therapy program into the total program.
Evaluate the job performance of Group Home personnel with the approval of the Residential Director.
Provide individual supervision for each Group Home Worker.
Define and begin to remedy any inherent problems in the resident/Group Home Worker relationship.
Plan and implement staff meetings for Group Home Workers.
Coordinate with the Group Home Workers for suitable and appropriate times for outside therapy and consultation when warranted for the residents.
Ensure the maintenance of agency policies and procedures and the Department of Human Services Rules
HOUSE SNAP/SOCIAL SUPPORT & MEDICALS
Maintain appropriate records of all SNAP applications and approved ones and ensure regarding all purchases are recorded and maintain such purchases in accordance with allotted funds.
Maintain appropriate records of all SS Benefits applications and approved ones and ensure regarding all usage are recorded and maintain such room and board payments are used with allotted fund
Coordinates client medical appointments and ensures that appointment outcomes and documentation are communicated to appropriate parties (ex-medication changes, changes in health status, special orders, follow-up appointments, etc
DIRECT SERVICES
Establish and follow through with a treatment plan for each resident.
De-escalate any peer to peer or peer to staff conflict
Plan and Coordinate Staff Training on behavior plans as already trained by the behavior of therapist and ensure that it is maintained.
Provide and ensure required casework services to each resident.
Work with outside agencies in the establishment of both short-term and long-term treatment plans for the residents.
Coordinate all transportation, medical, therapy, school, volunteer, and parental activities.
Relate to the residents in a supportive manner
Provide supplemental services to the Group Home Worker as is required in the daily encounters with residents.
Assist to Provide 24-hour back-up services as required.
Provide supervision to interns.
PROGRAM ADMINISTRATION
Responsible for hiring and terminating Group Home personnel, with approval of the Residential Director.
Plan and implement Group Home programs/ Day Care/Job and Training as well as visits outside the home to include individual, family and group counseling and behavioral and house support.
Plan, implement and supervise Group Home routine.
Complete and submit attendance records for all Group Home personnel.
Maintain records and the required data for each resident.
Carry out all duties as designated by the Residential Director.

COMPETENCIES:
Education:
BA in Social Work /Human Services or Human Relations or field related to developmental disabilities or
Required Knowledge/Skills/Abilities:
i. Read, write and understand the English language.
ii. Able to establish and maintain effective relationships with consumers, co-workers, neighbors, volunteers, vendors and the general public.
iii. Possess high degree of patience and the ability to tolerate frustrating situations.
iv. Knowledge of HIPPA Guidelines and Patient Privacy required.
v. Current CPR/First Aid certification is mandatory.
vi. Able to multi-task and manage work time efficiently.
vii. Able to see, talk, hear, walk, bend, stoop, sit, stand, reach, and lift items weighing 30 pounds or less repeatedly during a shift.
