Summary

The Family-Based Intensive Treatment (FIT) Clinician is responsible for working as a member of a 3-person, multidisciplinary team to offer a combination of intensive family therapy, care coordination and Family Partner support for youth under 21 who are at increased risk for hospitalization and meet other Medical Necessity Criteria (MNC). They are responsible to the Clinical Team Lead and the CSA Program Director.

Job Responsibilities & Essential Functions

  • Complete a comprehensive home-based assessment with the family in collaboration with the referral source and existing providers.
  • FIT clinicians become certified in the Massachusetts CANS Assessment and the CRAFFT Assessment
  • Complete an initial CANS-MA assessment and CRAFFT assessment (for youth 12 and older).
  • Completes initial and ongoing risk assessment and safety planning. The safety plan is reviewed regularly and updated as needed. Develops an initial treatment plan right after intake to address immediate needs and concerns.
  • A FIT clinician meets with a family 3-5 times a week, as clinically needed and decided upon by he family and the FIT team. Sessions are expected to be in the home or community, in a location deemed safe and preferable by the youth, family, and provider.
  • Develops a formal treatment plan that incorporates established psychotherapeutic techniques and intensive family therapy to implement structured interventions and behavioral techniques. Treatment plan is reviewed/updated at a minimum, every 30 days.
  • Completes consultation and collaboration with supervisors, other members of the FIT team, outpatient providers and other active parties to help involved in the youth and family’s treatment.
  • Maintains ongoing collateral contact with other service providers, state agencies involved with the family and any other pertinent supports to the youth and family, as necessary.
  • Ensures that all direct services and collateral contact is documented in the youth’s EHR.
  • Provides care coordination as clinically indicated via phone calls, in-person meetings and virtual meetings with collaterals for the duration of enrollment in FIT services.
  • Supports the youth’s attendance at school, as clinically appropriate, including collaboration with therapeutic supports available at the school.
  • When an enrolled youth is involved with crisis and/or subsequent placement in a higher level of care, the clinician maintains communication with the family and the admitting facility, providing treatment recommendations in attending treatment meetings as clinically needed.
  • Using the treatment plan, identify and actively assist the family to obtain and monitor the delivery of available and agreed upon services.
  • When requested by the family, and when clinically appropriate, attends and supports a youth and family at meetings related to the youth's behavioral health needs.
  • Promotes linkage with outpatient providers by assisting the youth and family in their attending outpatient appointments.
  • Engages in robust discharge planning, that begins ists the youth and family in assessing and accessing other levels of care as clinically appropriate.
  • Makes referrals to ongoing community-based support that will be necessary to the youth's treatment upon discharge from FIT services.
  • Updates the treatment plan, safety plan and comprehensive assessments at discharge.
  • Maintain consistent communication with supervisor and team members: remaining available via work cell phone and by reading and responding to work related emails daily.
  • Participates in ongoing program and agency meetings as required.
  • Remain up to date with all paperwork and documentation needs within the FIT program.
  • Performs other duties as required.
On-Call
  • FIT service providers are required to have a 24-hour urgent response system, available in person or by phone, depending on the clinical need. The clinician part of a rotating on-call system, sharing on-call responsibilities across the program.
