Center Well Home Health is seeking a Home Health Speech Therapist to evaluate and treat patients in their homes, directing speech-language pathology services and contributing to Plan of Treatment and Plan of Care.
You will assess disorders of speech, voice, language, hearing and swallowing; administer diagnostic tests; coordinate care with supervisors and physicians; provide counseling to patients and families; and maintain thorough documentation to support quality home-based rehabilitation.
