Alignment Health is seeking a compassionate, customer-service oriented remote Care Coordinator to join the outpatient case management team. You will work with the RN case manager and interdisciplinary team to support members with complex and chronic health needs, coordinating services, closing care gaps, and ensuring timely care.
This fully remote role requires at least one year in health care, strong communication and organizational skills, and preferably bilingual English/Spanish.
