Under the direction of the department manager, the Medical Assistant will be assisting the team in referral management for High-Risk patients and helping to assist with patient needs.

Receive new patient referrals from clients
Create charts for all new patients
Input medical information in new chart
Help Care Coordinators with patient needs as applicable
Foster relationships with their team to ensure the Care Harmony culture is positive and supportive
Other duties as applies
Required Skills/Abilities:Great attention to detail
Excellent organizational skills
Excellent verbal and communication skills
Excellent professional presence when dealing with colleagues
Excellent time management skills with a proven ability to meet critical deadlines
Fundamental Skills with Microsoft Office Suite
RequiredEducation and Experience:
Medical Assistant certificate must be active and in good standing
Physical RequirementsThis position is sedentary and will require sitting for long periods of time
This position will require the ability to speak clearly and listen attentively, often by telephone, for an extended period of time
The position will require the ability to understand, process, and take thorough notes in real-time on telephone conversations
