Vaya Health is seeking a Diversion Specialist to support the DRS project, conducting screenings and case management for individuals at risk of entry into Adult Care Homes. This mobile role requires travel and substantial field work across NC communities.
You will interact with hospitals, DSS, ACHs, and health providers to promote person-centered care and ensure timely documentation and coordination with TCL teams.
