Concert Health is seeking a Collaborative Care Clinician II in Massachusetts for a fully remote role. You will provide evidence-based interventions within the Collaborative Care Model to pediatric and/or adult patients, coordinating with primary care and other providers to support symptom reduction and improved outcomes.
The role requires a Master’s in Social Work, LICSW (MA) or equivalent, and at least 2 years in pediatrics, with fluent English/Spanish interpretation.
