Elara Caring is seeking a part-time Clinical Team Specialist to support high-quality home health care remotely across our patient network. You’ll coordinate patient care, manage plans of care, and ensure timely visits while maintaining compliance.
The ideal candidate holds an active OT or PT license (MA preferred) with 2+ years in home health, Oasis experience, and proficiency with Homecare Homebase and Workday. Travel may be required to support field offices as needed.
