Job Description & How to Apply Below

God’s Love We Deliver, a nonsectarian organization, is the New York metropolitan area’s leading provider of life-sustaining meals and nutrition counseling for people living with severe illnesses. Begun as an HIV/AIDS service organization, God’s Love provides for people living with more than 200 individual diagnoses. God’s Love cooks and home delivers the specific, nutritious meals a client’s severe illness and treatment so urgently require.

Meals are individually tailored for each client by one of our Registered Dietitians, and all clients have access to unlimited nutrition counseling. God’s Love supports families by providing meals for the children and senior caregivers of our clients. All of our services are provided at no cost to our clients.

About the Role:

God’s Love We Deliver has partnered within the healthcare field since our inception. The Business Development and Community Partnerships Department establishes partnerships and contracts with organizations that include managed care providers, hospitals, health systems, and community-based organizations.

The Community Outreach Associate is responsible for conducting outreach, engagement, and education sessions to cultivate consistent referrals from a range of healthcare and community providers. The role supports the team’s efforts to increase awareness of God’s Love, expand the number of clients served through existing and new partnerships, and grow the organization’s network of healthcare and community partners. The Community Outreach Associate will use research and data to engage partners, build and strengthen relationships, conduct outreach to prospective and current partners, and generate client referrals that support annual goals for clients served and revenue.

This is a remote position that requires travel and flexibility throughout the New York Capitol Region. Your office will also include community and healthcare spaces. You must be a New York Capitol Region resident. Home visits are a possibility. Your leadership and team will be in New York City. Must be a self-starter and comfortable reporting daily.

Responsibilities:
  • Conduct outreach and engagement with all current and potential partners, including managed care organizations, healthcare systems, federally qualified health centers, community-based organizations, community groups, and other referral sources, through meetings, presentations, and other engagement activities.
  • Develop, strengthen, and execute outreach, engagement, and education strategies to reach current and prospective referrers to the philanthropic program, including Ryan White-funded programs serving people living with HIV, other grant-specific initiatives, and contracted healthcare and managed care partners.
  • Develop and execute outreach strategies that advance the organization’s health equity goals by connecting with individuals and communities historically marginalized and disproportionately impacted by chronic disease.
  • Deliver high-quality customer service and maintain strong relationships with partner organizations, providers, and clients. Support the team in responding to partner needs and strengthening engagement with referral agencies.
  • Partner with the Chief Business Development Officer, colleagues across the Business Development & Community Partnerships Department, and other client-facing Managers and Directors to coordinate outreach activities, ensure appropriate coverage, and track meetings, presentations, and other engagement efforts.
  • Maintain consistent communication with staff at community-based and partner organizations to coordinate education sessions and teleconferences for current and new agency staff. Engage providers on referral trends, program updates, and service offerings, and develop presentations and educational materials that increase understanding of medically tailored meals and medical nutrition therapy for individuals living with chronic illness.
  • Partner with the Manager of Healthcare Partners and Manager of Client Services to facilitate new enrollments resulting from outreach and partnership activities.
  • Maintain accurate records of outreach, engagement, and marketing activities, including…
