purpose
The WIC Manager/Nutritionist is responsible for the nutrition and breastfeeding promotion for WIC participants at Fair Haven Community Health Center (FHCHC).
Duties and responsibilitiesReporting to the Director of Programs, the WIC Site Nutritionist collaborates with FHCHC Departments (i.e. Healthy Start, Midwifery, Pediatrics, Care Coordination, etc.), the Yale New Haven Health WIC Department, and community agencies to provide WIC services to the Fair Haven catchment area.
Typical duties include but are not limited to:
- Provides daily oversight of the FHCHC WIC program as prescribed by the CT DPH WICDepartment and Federal and State WIC Regulations. Creates/maintains the WIC schedule to include mandatory trainings (FCHC and WIC), PTO days, and staffing challenges. Communicates with Yale New Haven Health contractors/CT DPH as appropriate. Coordinates all technology needs, brochures, and educational materials from CT DPH.
- Responsible for the professional development and performance management of the FHCHC WIC Team including annual performance evaluations, annual training requirements, maintaining HR policies and procedures, and PTO schedules. Adherence to the Remote Work Schedule per HR.
- Manages and facilitates monthly staff meetings, daily communications, and disseminates pertinent WIC information to team. Attends WIC meetings as appropriate.
- Contributes to the FHCHC community by attending management meetings, providing WIC metrics, contributing to newsletters, forums, etc.
- In cooperation with the program Nutritionists and local agency Breastfeeding Coordinator develops and implements the nutrition and breastfeeding promotion and support services component of the annual local agency plan in compliance with Federal and State regulations and guidelines.
- Assures proper certificationof eligible applicants. As a Competent Professional Authority (CPA) on the staff of the local program, conducts a complete WIC nutrition assessment, determines and documents nutritional risk of WIC participants; as appropriate develops and follow-ups on care plans for high-risk participants. Is responsible for appropriate assignment and tailoring of WIC food packages for WIC participants.
- Overseesdistribution of WIC food benefits and farmers market checks to participants and maintains safeguards for WIC checks (equipment).
- Prepares & submits required reports and budgets to New Haven WIC Program Coordinator and/or FHCHC WIC Site Manager.
- Represents FHCHC and the WIC Program at meetings and CEU programs conducted by State and local WIC Programs.
- Performs other necessary duties are required by the Fair Haven Community Health Care to achieve the goal of providing quality nutrition and breastfeeding education and support.
- Works collaboratively with the Director to build alliances and expand the work of the Department.
- Master's degree from an institutionaccredited by a recognized regional accreditingbody in nutritionalsciences, community nutrition, clinical nutrition, dietetics, public health nutrition, home economics with a major in foods and nutrition and one year of responsible experience in nutrition in a health agency or health care facility, OR
