Shared Wellness is seeking dedicated LPNs to provide home-based nursing for an individual who is Trach, G-Tube and Vent dependent. Shifts are 12 hours (08:00–20:00 or 20:00–08:00). Both full-time and part-time roles are available.
The ideal candidate holds a PA LPN license, CPR certification, and has at least 1 year of homecare plus Trach/Vent/G-Tube experience, with strong documentation and compassionate, patient-centered care in home and community settings.
