Coordinate care for complex patients | Work from home Support high-risk and complex patients through medication reviews, care coordination, and adherence support — all from a remote setting. What You'll Bring

Education:

PharmD or Bachelor of Pharmacy.
Licensure:
Active U.S. pharmacist license.
Experience:

Case management, MTM, or managed care preferred. Hospital, ambulatory, and community pharmacists with strong counseling backgrounds are encouraged to apply.
