Clinical Pharmacist (Medicaid Population)

Are you a compassionate pharmacist who is passionate about improving health outcomes and helping underserved populations navigate the healthcare system? Join our team as a Clinical Pharmacist, where you'll have the opportunity to build trusted relationships with Medicaid patients across Missouri and play a vital role in their healthcare journey.

In this telehealth-based position, you'll serve as a dedicated advocate and clinical resource for patients who often face barriers to care. Through phone-based outreach and ongoing support, you'll help patients understand their medications, improve adherence, coordinate care, and connect with valuable healthcare and community resources.

What You'll Do

As a Clinical Pharmacist, you will:

+ Build rapport and trust with Medicaid participants through regular telephonic outreach.

+ Reintroduce enrolled patients to the program and engage them in ongoing care management services.

+ Serve as a pharmacist advocate and care coordinator throughout each patient's healthcare journey.

+ Collaborate with physicians, pharmacies, community health workers, caregivers, and case managers to ensure continuity of care.

+ Conduct medication reviews and medication reconciliation.

+ Provide patient-centered education on medications, disease states, and self-management strategies.

+ Support medication adherence by identifying barriers, discussing missed doses or refills, and developing individualized solutions.

+ Assess physical and behavioral health needs using established screening tools and clinical information.

+ Develop and monitor personalized medication treatment plans.

+ Connect patients with community resources, support services, and transportation assistance as needed.

+ Document interventions and outcomes in compliance with program requirements.

Medication Therapy Management (MTM) Services

Provide comprehensive MTM services designed to improve patient outcomes and promote medication optimization, including:

+ Medication adherence counseling and refill support

+ Comprehensive medication reviews (CMRs)

+ Medication education and therapy management

+ Chronic disease self-care education

+ Health status assessments

+ Development and monitoring of medication treatment plans

+ Evaluation of patient response to therapy

+ Identification, resolution, and prevention of medication-related issues

+ Communication of recommendations to prescribing providers

+ Coordination of diagnostic testing and follow-up care

+ Collaboration with community health workers and care teams

+ Connection to community-based support resources

+ Care coordination across providers, caregivers, and advocates

+ Transportation coordination for in-person healthcare appointments when necessary

Documentation & Compliance

Maintain accurate, timely, and compliant patient records, including:

+ Service dates and times

+ Patient identification information

+ Electronic verification or patient acknowledgment of services

+ Treatment plans, assessments, recommendations, and outcomes

+ Time spent delivering services

+ Clinical findings and follow-up plans

+ Communication with providers and care team members

Patient Care Coordination

+ Schedule and coordinate MTM appointments with patients.

+ Obtain required patient verifications following service completion.

+ Partner with healthcare providers, case managers, family members, and community organizations to support patient success.

+ Act as a central point of contact to ensure patients receive the services and support they need.

Qualifications Required

+ Current, active Pharmacist license in the State of Missouri.

+ Eligibility to enroll in the Missouri Medicaid program (if not already enrolled, enrollment must be completed prior to starting; there is no cost to enroll).

+ Strong communication and patient engagement skills.

+ Comfort working primarily by phone, email, and fax.

Preferred

+ Medication Therapy Management (MTM) experience.

+ Ambulatory care, managed care, population health, or insurance-based pharmacy experience.

+

Experience with SOAP note documentation.

+ Experience supporting patients with chronic disease management.

New graduates are encouraged to apply, as well as pharmacists with ambulatory care, managed care, community health, or insurance experience.

Job Type & Location

This is a Contract position based out of Saint Louis, MO.

Pay and Benefits

The pay range for this position is $50.00 - $55.00/hr.

Individual compensation offered for this position within this range will depend on many factors, including qualifications, skills, relevant experience, job knowledge, geographic location, internal equity, and other pertinent job-related factors.
