Liaison Specialist
Build meaningful connections across hospital and community care while helping people move forward with confidence, support, and continuity. This is a remote position open to Missouri Residents only.
The Liaison Specialist plays a key role in connecting patients with the care, resources, and follow-up services that support their health and well-being. Working as part of an integrated care team with Compass Health and SSM Health, this role helps strengthen continuity of care across hospital, primary care, behavioral health, and community settings. The Liaison Specialist builds relationships, coordinates referrals, supports health literacy, and helps identify and address barriers to care—all while honoring the strengths, goals, and individual needs of each person served.
This is a great opportunity for someone who enjoys building relationships, coordinating care across multiple systems, and helping people navigate healthcare and community resources. You'll collaborate with hospital staff, healthcare providers, community organizations, and other partners to help create connected, person-centered pathways to care.
