About the RolePosition: Physical Therapist
Our integrated care model pairs rehabilitative therapists with other clinical caregivers to help residents and patients achieve full recovery and a timely return to activities they enjoy.
Responsibilities- Collaborate with interdisciplinary team members to develop and implement individualized care plans.
- Provide therapeutic services to promote mobility, pain management, and functional independence.
- Document patient progress and adjust interventions as appropriate.
- Educate patients and families about treatment goals and recovery strategies.
- Graduate of an accredited physical therapy program with a minimum Bachelor of Science degree.
- Valid Physical Therapy license in the state where services are rendered.
