ALS United Ohio is a nonprofit organization seeking a CRC, OTA, PTA or Social Worker licensed in Ohio for a Care Services Coordinator role serving Ohio patients and families affected by ALS. You will guide patients through illness progression, connect with community resources, and coordinate clinic and program referrals.
The role requires strong communication, HIPAA confidentiality, and ability to work independently in a home-based setting with local travel to Columbus area and ALS clinics.
