Physical Therapist

Med-Scribe is partnering with a growing local physical therapy practice to identify a compassionate and experienced Physical Therapist who is passionate about helping older adults maintain their independence, mobility, and quality of life. This is an outstanding opportunity for a clinician who enjoys building meaningful relationships with patients while providing highly individualized, one-on-one care in the home setting. • Flexible schedule with a minimum commitment of just 2-3 days per week, Monday through Friday
• Primarily weekday hours between 9:00AM-3:00PM
• One-hour patient visits that allow for personalized care
• No insurance billing responsibilities
• Web-based EMR and streamlined administrative support

The Physical Therapist will provide in-home therapy services throughout the eastside of Rochester, working primarily with older adults experiencing deconditioning, balance deficits, mobility limitations, and other conditions affecting their ability to remain active and independent. Strong communication skills are essential, as therapists regularly collaborate with family members, caregivers, aides, and other members of the patient's care team.

• Providing skilled physical therapy services in patients' homes and select community settings
• Scheduling patient visits and managing follow-up appointments
• Communicating effectively with patients, families, caregivers, and care team members
• Completing evaluations, progress notes, and treatment documentation within 24 hours
• Utilizing therapeutic equipment and creative home-based exercise interventions to support patient goals
• $70 per patient visit
• Visits are approximately one hour in length
• Mileage reimbursement at 50% of the IRS rate
• Administrative support for patient intake and insurance verification

• Minimum of 2 years of physical therapy experience
• Current New York State Physical Therapist license
• Home care experience preferred; strong interest in geriatrics will also be considered
• Passion for exercise, wellness, and functional independence
• Strong interpersonal and communication skills
• Ability to work independently and manage a flexible schedule
