The Home Health Physical Therapist at Purpose Care is a full-time, field-based role in Cincinnati. This position is responsible for evaluating patients in their homes, developing individualized treatment plans, and providing evidence-based physical therapy interventions to improve mobility, strength, and functional independence. Day-to-day tasks include performing comprehensive assessments, educating patients and families on home exercise programs and safety, coordinating care with physicians, nurses, and other disciplines, and documenting services and progress in accordance with clinical and regulatory standards.

The Physical Therapist also monitors patient response to therapy, adjusts treatment plans as needed, and supports safe transitions of care to help patients remain confidently at home.

• Candidates should possess strong knowledge in Physical Therapy and Rehabilitation, including assessment, treatment planning, and evidence-based therapeutic interventions.
• Candidates should possess skills in Patient Care and Home Care, with the ability to deliver compassionate, person-centered services in a home setting.
• Candidates should possess a foundational understanding of Medicine as it relates to home health, including coordination with interdisciplinary care teams.
• Active state licensure as a Physical Therapist in Indiana, and graduation from an accredited Physical Therapy program.
• Experience in home health or community-based care is preferred, with proficiency in clinical documentation and use of electronic health records.
• Strong communication, time management, and problem-solving abilities, with reliable transportation for home visits.
• Ability to work independently, adhere to safety and infection control protocols, and support diverse patients and families with respect and cultural sensitivity.
