Gentiva seeks an After hours RN Care Manager to provide high-quality hospice care outside standard business hours. You will perform assessments, visits, and care coordination for patients and families, acting as clinical lead during nights, weekends, and on-call shifts.
You will collaborate with physicians and the IDG, maintain documentation, and educate families on care plans while ensuring compliance and infection control in a home-based hospice setting.
