VNA HEALTH AT HOME Home Health Aide – Job Summary

Home Health Aides assist patients with personal care, light housekeeping, and support tasks as outlined in the patient care plan. They collaborate with registered nurses and other team members to ensure safe, respectful, and high‑quality care.

• Assist with bathing, oral hygiene, dressing, transfers, ambulation, toileting, and feeding.
• Perform light household services essential to patient care.
• Report any recognized changes in patient condition to the RN case manager or designee.
• Document care provided using agency mobile device or other approved methods.
• Adhere to agency policies, state regulations, and the patient care plan.
• Provide a positive care experience for patients assigned.
• Participate in required inservice training and attend at least 12 hours annually.
• Perform additional duties as determined by management.

• Successful completion of a state‑approved Home Health Aide, CNA, or Nursing Assistant program; or a student nurse with a minimum of 75 hours of theoretical and clinical instruction.
• Valid driver’s license and reliable personal vehicle.
• Physical, mental, and sensory abilities sufficient to meet performance criteria with or without assistive devices.
• Ability to drive to patient homes and traverse various weather and environmental conditions.
• Commitment to maintaining patient safety and confidentiality.

Shift duration: 8 hours. Work settings include nursing homes, private residences, and other community environments. Duties may be performed indoors and outdoors as required.
