The Team Support to Improve Glycemic Control Using CGM in Diverse Populations (TEAM CGM) research study is seeking a bilingual (English/Spanish) Community Health Worker (CHW) to support study participants in the greater Worcester and Springfield areas. This per-diem position requires approximately 5-10 hours per week and includes telehealth patient visits, documentation, study meetings, and occasional local travel for in-person home visits when needed.

Compensation is $24-$27 per hour, depending on experience. Applicants should have at least 1-2 years of relevant experience as a Community Health Worker, Patient Navigator, Health Coach, or in a similar role.

About the TEAM CGM Research Study:

The TEAM CGM research study is funded by the National Institutes of Health (NIH) and conducted through UMass Chan Medical School. The study team is researching whether support from clinical pharmacists and community health workers, combined with telehealth and continuous glucose monitoring (CGM), can improve blood sugar control and diabetes care.

CHW Responsibilities:
  • Facilitate telehealth visits between patients and pharmacists using Zoom or phone.
  • Assist patients with medication reconciliation and discuss medication adherence concerns.
  • Reinforce and clarify pharmacist recommendations and care plans.
  • Support patients with continuous glucose monitoring (CGM) procedures and troubleshooting.
  • Provide diabetes education and self-management support.
  • Document patient interactions according to study procedures and communicate relevant information to the study team.
  • Enter and maintain study encounter data in the REDCap data management system.
  • Schedule and coordinate telehealth, home-based, or clinic-based study visits.
  • Assist patients with patient portal enrollment, authentication, and use.
  • Encourage patients to use the patient portal to request prescription renewals, schedule appointments, and communicate with their care team.
  • Attend monthly case review meetings with pharmacists and other CHWs.
  • Attend quarterly meetings with the study team.
  • Complete all required study and telehealth training.
Qualifications:
  • Bilingual in English and Spanish required.
  • Comfortable conducting telehealth visits and as-needed in-person home visits required.
  • Reliable transportation and ability to travel locally within the Worcester and Springfield areas required.
  • Experience working with underserved communities preferred.
  • Knowledge of diabetes or chronic disease management preferred.
