Conduct comprehensive medication reviews, coach patients on adherence, and improve outcomes — often from home or with flexible hybrid schedules.

key responsibilities

• perform comprehensive medication reviews (cmrs) and targeted medication reviews with patients.
• identify medication-related problems, drug interactions, adherence barriers, and cost-saving opportunities.
• collaborate with prescribers and care teams to resolve therapy issues and document interventions.
• educate patients and caregivers on safe, effective medication use.
• maintain compliance with cms mtm program requirements and quality metrics.

what you'll bring

• education: pharmd or bachelor of pharmacy.
• licensure: active u.s. pharmacist license.
• experience: ambulatory, hospital, community, or prior mtm experience is a plus. all licensed pharmacists, including new graduates, are encouraged to apply.
• skills: excellent patient counseling, independent work style, and clear documentation.
