Location: Northern
United Health Group is seeking Wellness Coordinators in a remote multi-state role to provide care coordination and administrative support to members with behavioral health and substance abuse needs. They will locate and engage members, screen for gaps in care, address social determinants of health, and develop individual care plans to overcome barriers to access to care.
