Visiting Nurse Services in Westchester seeks a Medical Supply & Patient Care Coordinator to support safe, efficient home care. This role coordinates medical supplies and equipment for home-based patients, verifies orders, tracks deliveries, and troubleshoots issues with vendors. The Coordinator assists clinicians with care planning logistics, updates EMR with supply and service details, and communicates with patients, caregivers, and the interdisciplinary team to ensure timely, accurate support.

This position helps maintain inventory, supports quality and compliance, and contributes to a mission-driven, patient-centered environment.

Responsibilities

  • Coordinate ordering, delivery, and tracking of medical supplies and equipment for home-care patients.
  • Verify provider orders and ensure accurate documentation in the EMR.
  • Communicate with patients, caregivers, clinicians, and vendors to resolve supply issues promptly.
  • Maintain and reconcile inventory of medical supplies and equipment.
  • Support clinicians with care logistics, including scheduling and follow-up needs related to supplies.
  • Ensure compliance with HIPAA and agency policies in all patient-related communications and records.
  • Assist with insurance or authorization information related to supplies when needed.
  • Monitor service quality and escalate concerns to leadership or clinical staff as appropriate.
  • Generate basic reports on supply usage, costs, and trends for leadership review.
  • Contribute to a collaborative, patient-centered, mission-driven care environment.
