Center Well Home Health is seeking a licensed Physical Therapist Assistant to deliver home-based therapy under the supervision of a Physical Therapist. You will administer treatment plans, perform therapeutic exercises, document care, and participate in team in-services to support geriatric patients in regaining strength and independence.
The role requires a valid state PT license, CPR certification, a valid driver's license with reliable transportation, and fluency in English.
