As a QA Nurse at Gentiva, you will ensure the highest standards of quality and compliance for our home-based healthcare services. You will review clinical documentation, monitor regulatory and policy adherence, and support continuous improvement across home health and hospice programs. Working closely with field clinicians and interdisciplinary teams, you will identify gaps, provide education and feedback, and help drive best practices in patient care and documentation.

Responsibilities
- Conduct clinical quality audits of home health and hospice patient records to ensure compliance with Gentiva policies, CMS and state regulations
- Review care plans, OASIS/assessment documentation, and medication records for completeness, accuracy, and timeliness
- Identify quality gaps, trends, and risk areas; prepare clear reports and recommendations for improvement
- Collaborate with field nurses, therapists, and interdisciplinary teams to reinforce best practices and resolve documentation issues
- Support performance improvement initiatives, including chart reviews, root-cause analysis, and corrective action follow-up
- Provide education and feedback to clinicians on quality standards, coding, documentation, and clinical protocols
- Assist with accreditation, regulatory surveys, and internal readiness reviews
- Maintain accurate QA logs, metrics, and dashboards to track quality outcomes and compliance
- Participate in policy and procedure updates related to quality and clinical practice
- Uphold Gentiva's culture of compassion, patient dignity, and clinical excellence in all QA activities
