Social Worker, Clinical Social Worker

As a Social Worker at Gentiva, you will provide compassionate, home-based psychosocial support to patients and families across our Healthcare & Medical Services programs. You'll conduct assessments, develop care plans, and coordinate community resources to address emotional, social, and practical needs. Working closely with an interdisciplinary clinical team, you'll advocate for patient dignity, facilitate care transitions, and support coping through counseling and education.

Responsibilities

• Conduct comprehensive psychosocial assessments for home-based patients and families.
• Develop and implement individualized care plans in collaboration with the clinical team.
• Provide counseling, crisis intervention, and emotional support to patients and caregivers.
• Coordinate community resources, benefits, and support services to address social determinants of health.
• Advocate for patient dignity, rights, and preferences in care planning and decision-making.
• Collaborate with nurses, physicians, and therapists to support safe care transitions and discharge planning.
• Maintain accurate, timely documentation in electronic health records and comply with regulatory standards.
• Educate patients and families on available services, advance care planning, and coping strategies.
• Participate in interdisciplinary team meetings and case conferences to review patient progress.
• Contribute to a culture of compassion, respect, and clinical excellence aligned with Gentiva's mission.
