Social Worker, Patient​/Health Advocate

As a Social Worker at Gentiva, you will provide compassionate, home-based support to patients and families receiving home health and hospice services. You will complete psychosocial assessments, develop care plans, and connect patients with community resources and benefits. Working closely with an interdisciplinary clinical team, you will support coping, advance care planning, and end-of-life decisions while advocating for patient dignity and preferences.

This role involves home visits, thorough documentation, and active participation in team meetings, all within a mission-driven, supportive culture focused on clinical excellence and empathy.

Responsibilities

• Provide psychosocial assessments and care planning for home-based patients and families
• Coordinate community resources, benefits, and support services to meet patient needs
• Collaborate with nurses, physicians, and hospice/home health teams to support care goals
• Educate patients and caregivers on coping strategies, advance care planning, and options of care
• Advocate for patient dignity, rights, and preferences throughout the care continuum
• Maintain accurate, timely documentation in accordance with regulatory and agency standards
• Participate in interdisciplinary team meetings to review cases and update plans of care
• Support crisis intervention, grief support, and end-of-life decision making as appropriate
