Harmony Cares is seeking a Licensed Clinical Social Worker to deliver compassionate, home-based care to medically complex and home-limited patients. In this role, you will conduct comprehensive psychosocial assessments, develop person-centered care plans, and provide counseling, crisis intervention, and ongoing support to patients and caregivers. You'll coordinate community resources and benefits, address social determinants of health, and collaborate closely with physicians, nurses, and other interdisciplinary team members.
This position blends in-home visits with telehealth and care management to improve outcomes, reduce avoidable hospitalizations, and enhance quality of life. Responsibilities
- Provide home-based clinical social work services to medically complex, home-limited patients and families
- Conduct comprehensive psychosocial assessments and develop person-centered care plans
- Deliver counseling, crisis intervention, and supportive therapy to patients and caregivers
- Coordinate community resources, benefits, and support services to address social determinants of health
- Collaborate with physicians, nurses, and interdisciplinary team members to optimize care
- Advocate for patients' rights, preferences, and access to needed services
- Document assessments, interventions, and care plans in the electronic health record
- Participate in case conferences, quality initiatives, and care coordination meetings
- Educate patients and families on coping strategies, advance care planning, and available resources
- Ensure compliance with ethical, legal, and regulatory standards in clinical social work practice
