Harmony Cares is seeking a Licensed Clinical Social Worker to provide compassionate, home-based support to medically complex, home-limited patients. In this role, you will conduct biopsychosocial assessments, deliver counseling and crisis intervention, and coordinate community resources to address social, emotional, and practical needs. Working within an interdisciplinary team of physicians, nurses, and care managers, you will help shape patient-centered care plans, support advance care planning, and document outcomes in our electronic health record.

Responsibilities
- Provide home-based clinical social work services to medically complex, home-limited patients and families
- Conduct biopsychosocial assessments and develop patient-centered care plans
- Deliver counseling, crisis intervention, and supportive therapy in the home and via telehealth
- Coordinate community resources, benefits, and social services to address social determinants of health
- Collaborate with physicians, nurses, and care managers in interdisciplinary team meetings
- Support advance care planning, goals-of-care discussions, and palliative care needs
- Document assessments, interventions, and outcomes in the electronic health record
- Participate in quality improvement and care coordination initiatives
- Educate patients and caregivers on coping strategies and community supports
- Maintain compliance with licensure, ethical standards, and organizational policies
