Choice Hospice, Inc. is seeking a Social Worker (MSW) to provide coordinated home-based hospice care in Scottsdale, AZ. The role involves psychosocial assessments, counseling, and referrals to meet patient and family needs.
Primary duties include completing assessments within 5 days of hospice election, assisting with advance directives and discharge planning, and collaborating with the IDG to coordinate care. A Bachelor’s in Social Work and hospice experience are required.
