#Jointhe Journey Position  Summary

The Memory Care Facilitator is responsible for planning, coordinating, and implementing person-centered programs and activities that support the cognitive, emotional, physical, and social well-being of residents living with Alzheimer's disease, dementia, and other memory-related conditions. This role collaborates closely with nursing staff, families, and interdisciplinary team members to provide meaningful engagement opportunities that promote dignity, independence, and quality of life.

Director of Activities, Life Enrichment Director, Executive Director, or Administrator

Essential Duties and Responsibilities Resident Engagement and Programming
  • Develop and facilitate daily memory care programs, activities, and therapeutic interventions tailored to residents’ abilities, interests, and needs.
  • Lead group and individual activities that stimulate cognitive function, social interaction, sensory engagement, and physical movement.
  • Utilize evidence-based dementia care practices to enhance resident participation and well-being.
  • Adapt programs and activities to accommodate varying stages of cognitive decline.
  • Encourage resident independence and meaningful choice whenever possible.
Person-Centered Care
  • Build positive relationships with residents and gain an understanding of their life histories, preferences, routines, and interests.
  • Incorporate resident backgrounds and personal experiences into activity planning.
  • Monitor resident responses to activities and adjust programming accordingly.
  • Promote a calming, supportive, and structured environment for residents.
Family and Resident Support
  • Communicate with family members regarding resident participation and engagement.
  • Encourage family involvement in memory care programming and special events.
  • Provide education and support regarding memory care activities and approaches as appropriate.
Collaboration and Teamwork
  • Work closely with nursing, therapy, social services, dietary, and other departments to support resident care goals.
  • Participate in care conferences and interdisciplinary team meetings as requested.
  • Assist in identifying residents who may benefit from specialized interventions.
Documentation and Compliance
  • Maintain accurate documentation of resident attendance, participation, and responses to activities.
  • Complete assessments and progress notes as required by facility policies and regulatory standards.
  • Ensure programs comply with state, federal, and organizational requirements.
  • Assist with department audits and quality improvement initiatives.
Environment and Safety
