Field-Based | Region 6, Louisiana | Pay Rate: $19.50 per hour

As a Medical Benefit Review Services Associate, you will support Louisiana Medicaid and long-term care programs by conducting in-home assessments, coordinating services, and helping older adults and individuals with disabilities access the healthcare and community resources they need to remain safely and independently in their homes.
This field-based position serves individuals throughout Avoyelles, Rapides, and Vernon Parishes and requires regular travel within the assigned region.
What You'll Learn and Do- Conduct in-home assessments for applicants and participants receiving Long-Term Personal Care Services (LT-PCS).
- Evaluate client needs and gather information to support eligibility determinations, care planning, and service coordination.
- Provide Choice Counseling for Home and Community-Based Services (HCBS) programs when requested.
- Educate applicants and family members about Louisiana Medicaid long-term care programs, healthcare services, and community-based support resources.
- Develop and maintain person-centered Plans of Care based on individual needs, goals, and functional assessments.
- Coordinate services and connect clients with healthcare providers, community organizations, and social service resources.
- Complete monitoring visits, client surveys, case documentation, and quality management activities.
- Maintain accurate records and ensure adherence to state and program guidelines.
- Receive specialized training in Louisiana Medicaid, long-term care, and community-based healthcare programs.
- Obtain IHC Certification through the Louisiana Department of Health.
- Gain hands-on experience in case management, care coordination, client assessments, and healthcare navigation.
- Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Public Health, Healthcare Administration, or related field.
- Strong verbal and written communication skills.
- Interest in working directly with diverse populations.
- Louisiana resident.
- Reliable transportation.
- Valid driver's license, current auto insurance, and ability to travel daily.
- Ability to work independently while managing multiple client cases and priorities.
