Center Well Home Health seeks a dedicated Occupational Therapy Assistant to deliver home-based therapy under the supervision of an OT. You will help patients regain strength, mobility and independence through rehabilitation and functional training in their own homes.
Responsibilities include implementing plans, designing adaptive equipment, guiding families on home programs, and coordinating care with the interdisciplinary team to support discharge and community reintegration.
